I have read and agree to the following terms and conditions:

SPECIFIC TERMS OF SERVICE: (Vehicle & Walking Tour Waiver)
I, the purchaser, agree to this release of all claims, waiver of liability and assumption of risk (“Waiver and Release”). I waive any and all claims I may have now and in the future, and release from all liability and agree not to sue My Chauffeur Transportation LLC., its officers, agents, representatives, employees or tour guides [hereafter referred to as “Staff”] for any personal injury, death, property damage, or loss sustained by me as a result of my participation in My Chauffeur Bus or Walking Tours due to any cause whatsoever, including without limitation, negligence on the part of My Chauffeur Transportation LLC., its Staff, tour partners, or other participants, or death or injury as a result of war, vehicles, civil unrest or epidemic. I further agree that if I violate this agreement and attempt to bring suit against any part of parties named herein, that I will be held responsible for attorney’s fees and any costs incurred by that person in defending such action.
I am aware that the tour, in addition to the usual dangers and risks inherent, has certain additional dangers and risks, some of which include: physical exertion for which I may not be prepared and weather or weather extremes subject to sudden and unexpected change. I accept all the inherent risks of the proposed bus or walking tour and the possibility of personal injury, death or property damage or loss resulting there from. I acknowledge that the enjoyment and excitement of the proposed form of tour is derived in part from the inherent risks incurred by travel and activity beyond the accepted safety of life at home or work, and these inherent risks contribute to such enjoyment and excitement, being a reason for my participation. I agree that if I suffer injury or illness My Chauffeur Transportation LLC. or its Staff can, at my cost, arrange any medical treatment and emergency evacuation service as it or they deem necessary for my health and safety.
I understand that photographs taken on this tour may only be used for non-commercial or personal purposes.
Further, I agree at all times during the tour, to abide by and comply with any additional instructions given by my tour guide or any other Staff and I accept that non-compliance may result in injury, death or permanent disability. I understand that non-compliance may result in my removal from the bus or walking tour.
In entering this Waiver and Release, I am not relying on any oral, written nor visual representations or statements by My Chauffeur Transportation LLC. or its Staff or any other inducement or coercion to go on the bus or walking tour. I confirm that, by my own free will, I have read and understood the Terms and Conditions of this bus or walking tour and Wavier and Release prior to signing it and agree that this Waiver and Release will be binding upon my heirs, next of kin, executors, administrators and successors.
I agree to grant the sponsors of this tour to a worldwide, royalty-free license to use my photographic, video, or digital likeness solely for promotional, educational, and/or commercial purposes.
As a participant on the tour, I accept full responsibility for my behavior, well-being, and health throughout the tour, which may involve a variety of activities, unfamiliar locations, and modes of travel, including but not limited to: climbing stairs. I further realize that the tour may involve walking through unfamiliar, narrow and/or steep corridors, gangways, roads, crosswalks, stairways and driveways.
By purchasing your reservation, I affirm that I have fully understood and agree to these Terms and Conditions, Waiver and Release of Liability.
UNIVERSAL TERMS OF SERVICE: (Main points of this form):

· No Smoking Please – Smoking of any kind is prohibited.
· SHUTTLE BUS - Do not stand up in the bus while it is moving.
· SHUTTLE BUS - Use emergency windows and roof exit ONLY in an emergency. PLEASE DO NOT TOUCH. Breakage fees apply. For ventilation, ask your driver.
· SHUTTLE BUS - DO NOT stand near the side (exit) door when the bus is moving. This rule is strictly enforced.
· SHUTTLE BUS - Do not leave carry-on baggage in the doorway or aisle.
· Drink alcohol moderately and responsibly – All ‘Hard Alcohol’ (15% ABV or higher) is PROHIBITED inside our vehicles due to over consumption/inebriation. Beer (6.4% or lower) or wine (14.9% or lower) are allowed but STRONGLY DISCOURAGED inside our vehicles due to over consumption. (Exception: Beer & Distillery Tour w/product purchased in route). Beer (6.5% ABV or higher) or wine (15% ABV or higher) are PROHIBITED. ALL ALCOHOL IS PROHIBITED IF NOT ALL PARTICIPANT’S ARE AGE 21 or over. Your Reservation Will End If Hard Alcohol Is Found. My Chauffeur is released & forever discharged from all responsibility related to damages, legal damages and/or injuries or death during and after this reservation caused from drinking alcohol.
· Sickness – Fines apply for sickness in or on the My Chauffeur vehicle. Please be pro-active & inform your driver.

I declare that I am 18 years of age or over and I sign with full agreement and understanding of the terms and conditions herein. I verify that I am entering into this agreement voluntarily and after careful consideration. I verify that My Chauffeur has recited items #1 - #3 above to our group.

- MASKS ARE RECOMMENDED TO BE PROPERLY WORN INSIDE THE VEHICLE AT ALL TIMES.
- VEHICLE WINDOWS WILL BE KEPT OPEN OR CRACKED FOR VENTILATION.

I have read and agree to the following terms and conditions:
- (Shuttle bus) - The emergency exits are in the roof and the 2 rear side windows with the red handles. To operate the roof exit in an emergency, turn & push the red knob. To unlock the window emergency exits; pull both red handles up and push out. The emergency roof exit and rear side windows are only to be unlocked and used in an emergency. Do not touch them unless there is an emergency. If you touch them and they break; you ARE held liable for repairs, replacement and/or vehicle down time up to $1000.00 per window or exit door + vehicle ‘down-time’ for repairs.
- (Shuttle bus) - This bus may start or stop suddenly. All passengers must initial the rental agreement document that My Chauffeur has instructed you to please remain seated while this bus is in motion. Due to the nature of this bus, and that it is a bus that allows standing, and that alcohol beverages are allowed on board, each passenger “rides at their own risk”. My Chauffeur will not be responsible for any passenger that injures his or herself in any way while aboard the vehicle or during the rental period. Therefore, please remain seated while the bus is moving.
- (Shuttle bus) - It is forbidden to be standing near the side exit door when the vehicle is moving. This rule is strictly enforced
- Food: For BYO food; please bring non-messy foods. My Chauffeur charges for food clean-up beyond the normal inside our vehicles See #11 Cleaning & Repairs under the service order agreement; below at bottom.
- All cleaning fees and damage fees will be paid for immediately and with no notice by charging your reservation agreement and a final price audit will be done after the conclusion of your reservation. - ALCOHOL: All hard alcohol (‘spirits’) is prohibited in My Chauffeur vehicles. Hard alcohol and/or empty hard alcohol bottles found in the vehicle will be subject to an immediate end of your reservation and/or fine in agreement with the contractual agreement (See Terms & Conditions #11). See FOOD & ALCOHOL, PART 1 on your reservation contract. Beer & wine are allowed* but not recommended due to over consumption. Purchased alcohol is requested to be put in the vehicle trunk. Passengers under 21 years of age may not consume alcohol but may be present if alcohol is present as long as they are accompanied by their parent.
